INCIDENT ALERT LOCATION COMPANY WIDE ALERT STATUS Normal ACTIVITY ACCESS amp EGRESS amp WORKING AT HEIGHT DATE ISSUED 11 02 2014 08 18 55 SUB ACTIVITY NA INCIDENT No 00376 TITLE Genie 2135 70 Mobile Elevated Work Platform MEWP ACCIDENT INCIDENT DETAILS Guidance issued by HSE An operator was killed when a Genie 2135 70 mobile elevating work platform became unstable and overturned whilst it was working at or very close to its maximum height In order to minimise the risks of overturn the machine safety devices and sensors must be configured as per manufacturer s settings as prescribed by the manufacturer in their Safety Bulletin 130005 dated 29th July 2013 HSE recommends confirmatory checks of the calibration before platform elevation Introduction The purpose of this safety alert is to advise owners and users of steps to be taken to control risks associated with the Genie 2135 70 Mobile Elevated Work Platform MEWP following preliminary findings for a machine overturn More information may follow in due course The emerging findings from the HSE investigations indicate that the operational stability of a Genie 2135 70 MEWP may be compromised by the incorrect measurement of the boom angle sensors Background In order to prevent the 2135 70 model machine operating outside its stability limits the onboard control system monitors the elevation of the secondary boom to prevent it from telescoping outwards until its el

3. closest to the pivot end as shown below Do not change the orientation of the digital level 7 Atthe ground controls a Start the engine b Fully raise the secondary boom a until any one of the inner boom sections just starts to extend c Stop extending and turn the engine off Note If the digital level times out before the secondary boom is fully raised repeat the procedure from step 4 8 Record the value displayed on the digital level in the attached Completion Form Ifthe value is greater than or equal to gt 75 5 the machine is safe to use and can be returned to service Finish filling out the Completion Form and return to Genie No further action is required If the value is less than lt 75 5 the machine must remain out of service The secondary boom angle sensor will need to be calibrated Continue on to step 9 3 of 6 Genie A TEREX BRAND 130005 EMEAR 9 Calibrate the secondary angle sensor of the machine per the instructions in the machine s service manual section 4 9 Record the desired digital level reading greater than or equal to gt 75 5 after calibration in the attached Completion Form Note Do not perform this procedure unless you are trained and qualified to do so To search for your nearest Terex AWP authorized dealer click the link below http www genielift com dealersearch 10 Return the machine to service 11 Finish filling out th

5. evation is sufficient as required by the manufacturer greater than or equal to 75 5 The MEWP s onboard control system relies on the correct setting of the elevation of each boom to maintain the effective operational stability of the machine Genie the manufacturer issued a Safety Notice on 29th July 2013 which required the immediate removal of all 2135 70 MEWP s from service until the listed inspections therein had been performed ACCIDENT INCIDENT IMAGES LEARNING POINTS ACTIONS TAKEN Action required The manufacturer s safety notice should be followed Any machines for which the required calibration checks have not been completed should be removed from service Following the completion of checks the machines should continue to be inspected serviced and maintained as per the manufacturer s requirements Work regarding the manufacturer s safety notice should only be carried out in accordance with the recommendations in the machine service manual i e on firm level ground with suitable environmental conditions etc Prior to any functional checks being undertaken by an operator using the platform controls as part of the manufacturer s listed inspections calibration of the angle sensors should be confirmed from ground level using an inclinometer The use of test weights might also be appropriate See attached additional pdf Relevant legal provisions e Sections 2 amp 3 of Health and Safety at Work etc Act 1974 e Regu
