Keep your database up to date Most data entry mistakes happen in the rush to update your information in time for quarterly reporting If you incorporate updating your database into the regular practice of serving your clients it will save you from any reporting deadline stress you will make fewer mistakes and in the long run you will spend less time updating the database e Back up your database lt takes a lot of diligence and hard work to keep the information in the Housing Help database up to date Don t let your hard work go to waste because of a technical glitch protect your work by backing up the database Make sure your organization has a plan for preserving the safety of your data

5. 1e cf 1 gt ouf gt of 1 Form View NUM WZ Entering Client Notes The database contains an area for you to enter your own notes on a client Notes are private and for you and your co workers reference only nothing you enter here will be submitted to the city To enter notes on a client select the Notes tab Here you will find a text box in which you can enter any information you choose up to 65 535 characters that s almost 12 000 words Updating an Existing Client s Intake amp Service Data If you have already created an intake and service data file for your client and wish to return to that file to update it you may do so by following these steps 1 Use the Go to File field to access the client s file You can either scroll down the list or search by typing the client s last name and then first name if the client shares a last name with other clients in your database Using the drop down box also allows you to see intake and discharge dates which is especially important in instances where a client has more than one intake file 2 If you need to delete existing details click on the information you wish to change to select it delete the details and then type new data to change a drop down option simply click on the preferred option and the original choice will be replaced 3 Once you have finished updating the file click the Save File button Screenshot 3 1 Access the client s file using
6. activate an existing application for housing Three of the four housing service categories contain both service start and service end dates only Eviction Prevention as a time limited crisis intervention does not require an end date The aim in recording these services is to capture the duration rather than the intensity of service You do not need to record every specific instance of service activities you provide but rather you will capture the length of time between beginning service and the resolution of that service Multiple services of the same category to the same client household A are entirely possible and should be recorded seguentially do not importa Yo open a new service until the previous service is closed for Housing Access and Housing Stabilization services However because you may help a client with several different housing applications at the same time you can have multiple Housing Applications started at the same time in a client s file N B Housing providers that maintain a waiting list for their housing may create a client file when the client initially applies for their housing Details such as date housed and date stabilization services commence should be completed when the client actually moves into this housing If the client finds other housing before a unit becomes available the client file should be closed with no date housed since the housing provider did not provide a unit within their own housing fo
7. database are confidential information The following suggestions should be implemented in compliance with your organization s policies on maintaining the confidentiality of client records e Every day you use the database adds information to the database so every day you enter new information you should also make sure the database is backed up e If your agency does not have a system for automatically backing up data on your computers you can back up the database by copying it to a removable media such as a CD DVD flash memory stick or tape a password protected medium is preferable e Before copying the database close it to make sure that no one is using it e Backup copies should be stored at a secure off site location e Backups should be checked periodically to make sure that the backup was successful and that the data can be recovered from the backup if needed Follow a rotational backup strategy keep backups from multiple points in time so that if problems develop in the database you can go back to the last good copy without losing too much of your information e If your organization is planning a move or upgrading your computers it is a good idea to make a backup of the database to a secure removable media 8 immediately beforehand as these sorts of changes are prime opportunities NO YS for your database to be accidentally deleted 24 Appendix A Glossary of Terms Intake and Discharge Data Case ID a numeri

10. of housing help services should get access to them However the possibility exists that a client may receive the same service from different service providers For clients this can mean relating their personal situation to housing workers at more than one agency To improve client services and maximize existing resources the City and housing workers must try to ensure that housing help services are coordinated services are easily accessible to the client efforts do not overlap and duplication of services or tasks is consciously avoided Therefore it is the responsibility of all housing workers to determine if new clients are working with another housing help service provider 1 If the client is not working with another housing worker the housing worker should initiate providing housing help services to the client 2 If the client is working with another housing worker the following guideline should be followed e Identify all services accessed by the client e Obtain written or verbal consent from client to communicate with the service providers e Case conference with service providers and preferably the client e Establish who will act as the primary coordinator of services for the client and whether there is a role for any other service provider 3 If it is established that there is not a logical role for all service providers then the client should be encouraged to decide which provider they would like to assist them Where

14. their housing as assessed by the housing worker or for other circumstances Discharge Referral For the primary referral established prior to discharge Indicate which type of program or supports to which the referral is made In the case of multiple referrals select as the primary referral the referral which will be most important in supporting the client household to keep their housing 1 None General Counselling Physical Health Services Mental Health Services Addiction Services Other Case Management Educational Training Employment Program Financial Literacy Trustee 10 Other Discharge Referral Agency Report the name of the agency or agencies to which the client was referred Service Data File ID a numeric identifier assigned automatically for each new case Client ID The Client ID is automatically generated by the database based on client information that allows the tracking of discrete clients without providing any identifying information to City of Toronto staff Service Type The types of housing help service provided to the client household The same household can receive these services multiple times while the case is active 1 Housing Applications helping a client household to submit a new application or renew or activate an existing application for housing 2 Housing Access the provision of services to assist households to obtain and move into housing 3 Housing Stabili

You can then type in the first few letters in the client s last name to move that point in the list 2 If the client household you are serving consists of more than a single individual you may also want to search to see if other members of the household are listed as clients in the database Decide which member of the household is your primary household contact for the client household and use her or his information for the database 3 If the client has previously been discharged the file will have a red background instead of the usual green For discharged clients there are two options e If the client s intake data is the same i e the household situation is the same amp housing demographic stats are unchanged you should re open the discharged intake See Reopening A Discharged Client File for instructions e If the client s intake data has changed create a new intake for the client You will not need to create a new client entry in the Client Form but you will need to complete a new intake with the new information 4 there is no existing intake for the client proceed to creating a new Client File Step C Create a new Client File 1 Click the Go to Client Form button or the Client button on the menu bar 2 Check if the client already exists by searching the Go to Client field The lack of an intake record for a client does not necessarily mean that the client has not been created in the Client form 3 Click on

18. ation counselling direct rent payment mental heath services and or addiction services on an ongoing basis or until the client household is ready to move to more independent accommodations For further information on supportive housing programs in Toronto please search for supportive housing at www 211toronto ca 3 Alternative Housing housing programs designated by the Social Housing Unit of SSHA as alternative housing providers In most cases these programs provide occasional or ongoing supports to tenants Alternative housing providers can take tenants off their own waiting list as well as the Housing Connections list 4 Homes for the Aged homes for seniors that are usually long term care facilities with private and or semiprivate accommodation Services may include health care and social and recreation programs Information on Toronto Long Term Care Homes and Services is available at http www toronto calltc and www 21 1toronto ca 5 VAW Violence Against Women programs that assist women and their children that are victims of violence to find affordable and or transitional housing For information on the application process and VAW housing programs please contact Housing Connections or specific VAW shelters and programs 6 Aboriginal Housing housing programs specifically for Aboriginal clients 7 Other any other not for profit or private housing 30 Appendix B Intake Guidelines for Housing Workers All clients in need

21. clients and the File form is for entering intake information and recording the services you provide This manual provides instructions to complete all the fields in both of these forms The manual is divided into three sections each section addressing a different part of your use of the database They are recording your clients data recording the services you provide to clients and managing your database While of course all information in this manual is important importat as you read through its pages you will occasionally come ibe across some information highlighted by an Important icon These icons are used to indicate information that is not only vital to the effective use and maintenance of your HHDb but also may be easy to overlook or not necessarily intuitive in your day to day use of the database Please take special notice of these points when using the HHDb This manual also includes three appendices of additional reference material e Appendix A Glossary of Terms a glossary of terms for the data template is included in this document as and is similar to the glossary available in the Performance Measures PM Guidebook e Appendix B Intake Policies e Appendix C Discharge Policies On the back page you will find a list of 12 tips for effective use of the database Finally should you have questions that are not answered by this manual please contact your lead City staff for further clarification or assis

24. fore quarterly reporting is a good time for this Even if a client discharge isn t appropriate you can still indicate that you are not actively providing a housing service by entering a service end date Don t forget to record an outcome or reason for end of service e Keep an eye on discharges t can prove a challenge to maintain proper caseload size information especially if your clients are known to disappear for lengthy stretches before returning for service Incorporate discharge planning into your updating of the database to make sure that your active client caseload is properly represented Don t be fooled by the FilelD number The File ID field is populated automatically to uniquely identify each file This is an important database function and does not affect the data you are collecting This field cannot be changed and it is not a file counter i e it should not be used to determine the number of files in the database Avoid overwriting client data When entering a new client or a new intake make sure that the form you are working in is blank by using the Create New Client and Create New File buttons If not you could be editing an existing file instead of creating a new file Be careful when editing data Databases automatically update information as you type Even if you don t click the save button any changes you make to the information will be preserved when you close the database program

30. ousing worker does not provide service y Housing worker provides service Worker has a role 32 Appendix C Discharge Guidelines for Housing Workers Not all clients need housing follow up services and case management once they are housed but some clients do For clients in need of these services it is the responsibility of the housing worker to either provide them or help their clients to connect to them This guideline helps to clarify housing workers role in this area Once an individual is housed and before they can be discharged as a client the housing worker who housed them has the responsibility to assess the client s need for housing stabilization services These services can range from housing follow up services to ongoing case management 1 If the client can manage independently with few supports already has supports in place or refuses support services the housing worker can discharge the client 2 If the client is in need of housing stabilization supports the housing worker at a minimum must make an effort to connect them with appropriate supports These support services can be provided by e any housing worker that assisted with housing the client e another staff from the same agency e another service provider through a referral process and or an informal network of family or friends Supports can be time limited or ongoing In all cases the service provider should work with the c

38. tance Recording Client and Intake Data Adding A New Client Adding a new client to the database is one task where an extra effort to avoid duplication proves very worthwhile Following these three steps when creating a new client file will help prevent duplicate records Step A Determine that the client is not receiving service from another provider All clients in need of housing help services should get access to them However the possibility exists that a client may receive the same service from different service providers See Appendix B Intake Guidelines for Housing Workers for guidance on intake practices It is also important that before entering client information into the database that you determine the members of your client household If the client is a single individual the answer is obvious but for client households with more than one adult the process of creating a client file should include the decision of who in the household you will record as the primary household contact for the purpose of record keeping Step B Make sure the client is not already in your records To avoid creating double entries for a client s files you should first check that the client is not already listed in your database You can do this in the File form by checking under the Go to File field the field with the yellow background 1 Click the downward pointing arrow on the right side of the Go to File field to access the drop down list

42. the self training presentation or try out a few scenarios Watch for changes in the client household composition If a client household has two adults and your primary household contact leaves the client household the original client file should be closed and a new file created with the new primary household contact Use a consistent date format Enter dates in a consistent format One of the most common mistakes in entering dates comes from reversing the day and the month If you consistently enter all dates in the format of DD Mmm YYYY that s two digits for day three letters for month and four digits for year e g 06 Feb 2010 you can help to prevent this mistake from happening e Intake data is for the whole household not just the primary household contact The information on the intake form does not apply to just the primary household contact but to any member of the client household Keep separate project data separate If your agency has more than one housing help project make sure you are recording data for the correct project by double checking the Project field located third from the top on the File form Remember to close off service When you enter an intake date for a service but not a service ended date it will appear that you are still providing service to that client Get in the habit of taking a moment to consider the clients you have served with whom you have not had recent contact be
