You will see these reports listed under the Subscribed Reports tab 4 3 1 To run a report 1 Choose the radio button of the report you would like to run and click on Configure at the bottom of the screen Version 1 4 5 0_rev 20 Apr 09 20 HOBIC User Manual 2 Depending on the setup of the report the configuration fields for each report may vary see 4 3 2 Report Descriptions and Configuration Options for details However you will always need to choose the format you would like the report to be in HOBIC Report 1 Mean Indicators by Unit Choose Report Format O PDF HTML e PDF you will need Adobe Acrobat to open the file e HTML the report will open in a new browser window 3 Enter the configuration information specific to each report see Report descriptions following this section 4 Click on Generate Report to run the report To cancel running the report click on Return to go back to the Subscribed Reports page If you have chosen HTML as the format a small window will pop up indicating the Terms of Use for running a report Please read carefully and click on OK if you agree to the Terms of Use Click on Cancel if you do not agree If you agree to the Terms of Use for running a report a new screen will open indicating that your request is being processed After the report has been processed it will open in this same window You can close the window when you are done with the report PDF format

9. see section above 3 3 2 Question types in the assessment you have various options to input your option a b c d 5 3 4 Clicking on any of the above buttons Save amp Resume Cancel or Submit for Scoring only Free text simply start typing in the field Pressing TAB at any point will move your cursor to the next question Checkboxes 1f you are on a question with checkboxes as options pressing the TAB key will move the cursor through all of the checkboxes If you want to select the option press Spacebar on your keyboard To de select a response press the spacebar again If you are on the last response option pressing TAB will move your cursor to the next question Drop down menus 1f you have chosen to view questions as drop down menus instead of radio buttons you can TAB to the field and then press the number of the option that you want to select In the example below you can press 1 on your keyboard to select In Hospital Pressing TAB at any point will move your cursor to the next question Unchecked Unchecked 0 AtHome 1 In Hospital Radio buttons if you have chosen to view questions as radio buttons you can use the up and down arrow keys on your keyboard and the space bar to select the value that your cursor 1s currently on Pressing TAB at any point will move your cursor to the next question Confirming navigation choices when the assessment is complete will bring you to

19. at the top of the page or the left and right arrow buttons at the bottom 5 3 2 Question types in the assessment The questions appear on the left hand side of the screen and the possible responses on the right hand side Depending on the question the response options can appear in several formats a Free text these are sections on the form that are open text areas that allow you to enter text These fields will all have a maximum number of characters that you can enter in the form Once you have reached this maximum you cannot add any more characters to the field There are two types text fields and text areas Text fields are one line only see Sent Home With below Text areas are multiple lines see Comments below Version 1 4 5 0_rev 20 Apr 09 34 55 HOBIC User Manual Sent Home With Comments b Checkboxes are separately listed options that have clickable squares beside them To choose a value click on the circle or the word of the response that you want You can choose MULTIPLE responses for a question of this type 0 Artificial Eye 1 Glasses _ 2 Contact Lenses c Drop down menus these are boxes with a downward facing arrow on the right hand side If you click on this box the menu will expand so you can see the options for that question Click on the response you want or you can enter the number value of the response you want for example to choose In Hospital in the example belo
20. e report without further categorizing the results by diagnoses Combine following diagnoses into one table Select Yes to combine all of the assessments with the entered diagnoses and display all of the data in one table Diagnosies Information Diagnosis G300 Alzheimer s disease with early onset O Include clients with diagnoses listed above Exclude clients with diagnoses listed above For the Acute Care sector the report is run using the discharge diagnosis For the other sectors HC LTC CCC the report is run using the admission diagnosis If your organization did not submit diagnosis information for a client they will be excluded from the report You can enter up to five 5 diagnoses to include in the report You can search by either diagnosis test or the ICD 10 code Once you have selected a diagnosis click on Add to include it in the report You can click on the Remove button beside each diagnosis to remove it from the report You must also indicate whether you want the report to Include or Exclude clients with the selected diagnosis by selecting the corresponding radio button Additional Options If you would like the report to just include O Include Composite Scores Only composite scores check the Include Composite C Include Report Description as Last Page of Report Scores Only box It this box is not checked the report will include all HOBIC measures as well as the composite scores You may also print a
21. e there is a Drop Down Menu that allows you to jump between pages in the assessment A brief description of each page is provided Version 1 4 5 0_rev 20 Apr 09 33 55 HOBIC User Manual i The Save amp Resume button will allow you to save any responses you have entered so far on the current page as well as any previous pages and come back to the assessment at a later time The Cancel button will return you to the assessment tool listing page If this is a new assessment all of the responses you entered into the assessment tool will be discarded including pre assessment information If you are editing a previously started assessment all the changes that you have entered since the last time the assessment was saved or submitted will be discarded me At any point during the assessment you can click on the Submit for Scoring button This will either submit the assessment if all mandatory fields have been completed or it will take you to the Summary page where missing mandatory elements will be listed Once you Submit an assessment you cannot re open it without manager approval There will be a right hand facing arrow on the first and subsequent pages to go to the next page After the first page there will be left hand facing arrow to return to the previous page E All of your responses that you have entered on each page will be saved as you navigate between sections using either the drop down
